Survey Setup
- Airport / facility identifier *
- Date of survey *
- Pavement feature (runway, taxiway, apron) and ID
- Pavement type (asphalt/AC or concrete/PCC)
Sample Units
- Were sample units defined per ASTM D5340 (AC ~5,000 sf, PCC ~20 slabs)? *
- Was a representative number of sample units surveyed for the branch/section? *
- Number of sample units inspected
Distress Survey - Flexible (AC)
- Were cracking distresses (alligator, block, longitudinal/transverse, reflection) recorded with severity?
- Were surface distresses (raveling, weathering, rutting, shoving) recorded?
- Were patching and jet-blast erosion recorded?
Distress Survey - Rigid (PCC)
- Were slab distresses (corner break, linear cracking, shattered slab, D-cracking) recorded?
- Were joint distresses (joint seal damage, spalling, faulting) recorded?
- Were popouts, scaling, and pumping recorded?
FOD & Rating
- Is the pavement free of foreign object debris (FOD) hazards and loose material? *
- Calculated / estimated PCI (0-100)
- Is maintenance or rehabilitation recommended for this section? *
- Notes and recommendations
