Program Structure

Is there a written JHA (or JSA) procedure defining when and how analyses are conducted? *
Are jobs prioritized for analysis by injury history, severity potential, and new or changed tasks? *
Are affected employees involved in developing and reviewing each JHA? *

Task Breakdown & Hazard Identification

Is each selected job broken into sequential basic steps? *
Are potential hazards identified for each step (struck-by, caught-in, chemical, ergonomic, fall)? *
Is the job observed as actually performed, not just as written? *

Controls

Are recommended controls selected using the hierarchy of controls (elimination to PPE)? *
Are engineering and administrative controls preferred over reliance on PPE alone? *
Are required PPE items specified for each hazardous step? *

Communication & Review

Are completed JHAs communicated to and used in training the affected employees? *
Are JHAs reviewed after incidents, near-misses, or process changes? *
Are JHAs reviewed on a defined periodic schedule (e.g., annually)? *
