Specimen & Patient Identification
- Is the patient positively identified using two independent identifiers at specimen collection? *
- Is the type-and-screen specimen labeled at the bedside with patient identifiers and collection date? *
- Is a second determination of ABO group required before issuing non-group-O red cells to a patient without prior history? *
Compatibility Testing
- Are ABO/Rh typing and antibody screening performed on the recipient before red cell transfusion? *
- Is crossmatch or electronic crossmatch performed and documented before red cell issue? *
- Are special processing requirements (irradiation, CMV-negative, leukoreduction) met when indicated? *
Storage & Issue
- Are blood products stored at monitored, alarmed temperatures with continuous recording? *
- Is a visual inspection of each unit performed before issue for abnormal color, clots, or leaks? *
- Is a final clerical check comparing the unit, tag, and patient identifiers performed before transfusion? *
Administration & Reactions
- Are patient vital signs recorded before, during, and after transfusion per policy? *
- Is transfusion of a unit completed within the maximum allowable time (typically 4 hours)? *
- Is a suspected transfusion reaction stopped, reported, and investigated with a clerical and serologic check? *
