Sort (Seiri)

  • Are only necessary items present in the work area, with unneeded items removed or red-tagged? *
  • Is a red-tag process in place for questionable items?
  • Are obsolete tools, materials, and documents removed? *

Set in Order (Seiton)

  • Does every item have a designated and labeled location? *
  • Are shadow boards, floor markings, or other visual controls used to indicate placement? *
  • Can needed items be located and returned within seconds?

Shine (Seiso)

  • Is the work area clean and free of dirt, debris, and leaks? *
  • Are cleaning responsibilities and schedules assigned? *
  • Is cleaning used as a form of inspection to detect abnormalities?

Standardize (Seiketsu)

  • Are visual standards (photos, layouts, standard work) posted for the first three S's? *
  • Are 5S responsibilities documented and assigned? *

Sustain (Shitsuke)

  • Are regular 5S audits conducted and scored? *
  • Are audit results posted and improvement actions tracked? *
  • Does management participate in and reinforce 5S?
  • Overall 5S score for this area
