Element Identification
- NBI structure number *
- Date of inspection *
- Prestressing type (pretensioned girders, internal PT, external PT, segmental)
- Are as-built tendon layout/duct drawings available for reference?
Concrete & Cracking
- Are prestressed girders/beams free of longitudinal cracking along tendon paths? *
- Are anchorage zones and end blocks free of cracking or spalling? *
- Are precast girders free of transverse cracks indicating prestress loss? *
- Is there rust staining along the soffit or web indicating strand corrosion?
Ducts, Grouting & Voids
- Are external tendon ducts (HDPE/steel) free of cracking, UV degradation, or splits? *
- Were grout voids or soft grout investigated at high points and anchorages where suspected?
- Was NDT (impact-echo, GPR, borescope) used to assess duct filling where warranted?
- Is there evidence of water intrusion into ducts at low points?
Anchorages & Deviators
- Are anchorage caps/pour-backs intact and sealed against moisture? *
- Are deviator blocks and saddles free of cracking or distress?
- Are exposed anchor heads, wedges, and strand tails free of corrosion? *
Assessment
- Element condition state (1-4)
- Is specialized NDT or a detailed tendon investigation recommended? *
- Notes and recommended follow-up
