Dam Identification & Classification
- National Inventory of Dams (NID) ID *
- Hazard potential classification (low / significant / high) *
- Is a current Emergency Action Plan (EAP) on file for significant/high-hazard dams? *
- Inspection date *
Embankment / Dam Body
- Is the crest free of settlement, cracking, or misalignment? *
- Are upstream and downstream slopes free of sloughing, bulging, or erosion? *
- Is there any observed seepage, boils, or wet areas on the downstream slope or toe? *
- Is animal burrowing or unwanted woody vegetation present on the embankment?
Spillway
- Is the principal spillway clear of debris and obstruction? *
- Is the spillway structure (concrete/weir) free of cracking, spalling, or undermining? *
- Is the emergency/auxiliary spillway intact and free of erosion or vegetation encroachment?
- Are spillway gates (if present) operable and maintained?
Outlet Works
- Is the low-level outlet / conduit operable and free of blockage? *
- Is there sediment/piping evidence at the conduit outlet (soil in discharge)? *
- Are outlet gates and valves exercised and functional?
Instrumentation & Monitoring
- Are piezometers, weirs, and monitoring points readable and reporting expected values?
- Reservoir/pool elevation at time of inspection (ft)
- Is instrumentation data trending within normal ranges (no anomalous pore pressure or seepage)?
Findings
- Are conditions observed that warrant emergency notification or drawdown? *
- Deficiency summary and recommended follow-up
- Inspector signature *
