Survey Setup

  • Airport / facility identifier *
  • Date of survey *
  • Pavement feature (runway, taxiway, apron) and ID
  • Pavement type (asphalt/AC or concrete/PCC)

Sample Units

  • Were sample units defined per ASTM D5340 (AC ~5,000 sf, PCC ~20 slabs)? *
  • Was a representative number of sample units surveyed for the branch/section? *
  • Number of sample units inspected

Distress Survey - Flexible (AC)

  • Were cracking distresses (alligator, block, longitudinal/transverse, reflection) recorded with severity?
  • Were surface distresses (raveling, weathering, rutting, shoving) recorded?
  • Were patching and jet-blast erosion recorded?

Distress Survey - Rigid (PCC)

  • Were slab distresses (corner break, linear cracking, shattered slab, D-cracking) recorded?
  • Were joint distresses (joint seal damage, spalling, faulting) recorded?
  • Were popouts, scaling, and pumping recorded?

FOD & Rating

  • Is the pavement free of foreign object debris (FOD) hazards and loose material? *
  • Calculated / estimated PCI (0-100)
  • Is maintenance or rehabilitation recommended for this section? *
  • Notes and recommendations
