Intersection Identification

  • Intersection / signal ID *
  • Inspection date *
  • Controller type / operating mode (pretimed / actuated / coordinated)

Signal Heads & Displays

  • Are all signal indications operating with correct color, sequence, and no burned-out modules? *
  • Are signal heads properly aimed, visible, and free of physical damage? *
  • Are backplates, visors, and mounting hardware secure?
  • Do phasing and clearance (yellow + all-red) intervals match the approved timing sheet? *

Controller & Cabinet

  • Is the controller cabinet weathertight, grounded, and free of corrosion or water intrusion? *
  • Is the malfunction management unit / conflict monitor present and passing self-test? *
  • Is the current timing/programming documentation stored in the cabinet?
  • Is battery backup / UPS (if equipped) functional?

Detection

  • Are vehicle detectors (loop/video/radar) calling and functioning on all approaches? *
  • Are any detector faults or continuous calls present? *
  • Are pedestrian and bicycle detectors functioning where installed?

Pedestrian Features

  • Do pedestrian signals display WALK / flashing DON'T WALK with correct clearance time for crossing distance? *
  • Are pushbuttons functional and, where required, accessible (APS)?
  • Inspector signature *
