Equipment Readiness

  • Are crash carts checked and documented each shift with an intact tamper seal? *
  • Is the defibrillator tested each shift and are pads/batteries in date? *
  • Are emergency medications on the cart current and matched to a standardized par list? *

Recognition & Activation

  • Is an early-warning or track-and-trigger tool used to identify deteriorating patients? *
  • Can any staff member, patient, or family member activate the rapid response system? *
  • Does the rapid response or code team respond within the facility-defined target time? *

Performance & Documentation

  • Is high-quality CPR (rate, depth, minimal interruptions) and defibrillation performed per AHA guidelines? *
  • Is a complete code record with timed interventions and medications documented? *
  • Do responders maintain current BLS/ACLS certification as required for their role? *

Review & Improvement

  • Is a post-event debrief conducted after code and rapid response events? *
  • Are code and rapid response events and outcomes reviewed by a resuscitation committee? *
  • Are time-to-defibrillation and survival-to-discharge metrics tracked?
