Pre-Inspection Documentation

  • Is the current OCIMF Vessel Inspection Questionnaire (VIQ) and pre-inspection questionnaire completed and up to date? *
  • Are the Photo Booklet and vessel particulars submitted through the SIRE 2.0 digital platform? *
  • Are class certificates, statutory certificates, and the ISM/ISPS documentation current and readily retrievable? *
  • Are open observations from the previous SIRE inspection closed out with evidence? *

Crew Competency and Human Factors

  • Can watchkeepers demonstrate knowledge of ship-specific procedures relevant to their assigned questions? *
  • Are matrix-of-responsibility and familiarization records complete for all officers and ratings? *
  • Are records of drills, toolbox talks, and risk assessments available to evidence a safety culture? *
  • Enter the number of days the senior officers have served together on this vessel.

Hardware and Process Verification

  • Are safety-critical equipment and alarms (ESD, gas detection, IG, PV valves) tested and functioning? *
  • Are cargo, ballast, and mooring operations supported by documented plans and permit-to-work controls? *
  • Is the vessel free of visible defects, leaks, and housekeeping deficiencies in accommodation and machinery spaces? *
  • Are Management of Change records available for any modifications or temporary arrangements? *

Inspection Conduct and Follow-Up

  • Is a senior officer nominated to accompany the SIRE 2.0 inspector throughout the inspection? *
  • Is the vessel prepared to demonstrate operations under conditions found rather than staged demonstrations? *
  • Attach a photo of the primary cargo control room readiness for the record.
