Building Identification and History

  • What is the approximate construction era and governing code vintage? *
  • What is the primary lateral system (wood, URM, tilt-up, steel, concrete)? *
  • Are original or retrofit structural drawings available for review?
  • Is the building in a mandatory retrofit jurisdiction or ordinance area?

Common Seismic Vulnerabilities

  • Is a soft-story or weak-story condition present (open front, tuck-under parking)? *
  • Is unreinforced masonry (URM) bearing or infill present? *
  • Are non-ductile concrete frames or inadequate detailing evident?
  • Are tilt-up or precast wall-to-roof anchorage deficiencies apparent?

Prior Retrofit and Structural Condition

  • Is documented evidence of prior seismic retrofit present (anchors, braces, shear walls)? *
  • Are foundations free of significant settlement, cracking, or displacement? *
  • Are structural members free of deterioration, corrosion, or prior damage? *
  • Photo of representative structural or retrofit condition

Non-Structural Hazards

  • Are parapets, cornices, and exterior appendages braced or anchored? *
  • Are heavy mechanical units, tanks, and equipment seismically restrained? *
  • Are suspended ceilings, light fixtures, and partitions adequately braced?
  • Are gas shutoff, piping, and utility connections protected against movement?

Risk Rating and Recommendations

  • What screening-level seismic risk rating applies (low, moderate, high)? *
  • Is a detailed ASCE 41 evaluation by a structural engineer recommended? *
  • Are probable maximum loss (PML/SEL) or insurance implications noted?
