Eligibility and Details
- Property address *
- Year the housing was built *
- Property is target housing (pre-1978 residential; not exempt) *
- Children under age 6 or a pregnant occupant reside in the unit
Paint Condition
- Deteriorated paint (peeling, chipping, chalking, cracking) was observed *
- Deteriorated paint present on friction/impact surfaces (windows, doors, stairs) *
- Total deteriorated paint exceeds the de minimis thresholds
- Photo of each area of deteriorated paint
Testing
- XRF or paint-chip testing was performed on suspect components
- Any component tested positive for lead-based paint (>= 1.0 mg/cm2 or 0.5% by weight)
- Dust-wipe samples were collected on floors and window sills
- Components tested and results
Hazard Controls and Documentation
- Bare soil in play/dripline areas was evaluated for lead hazards
- Firm/renovator performing disturbance is EPA Lead-Safe certified (RRP) *
- Lead disclosure (and EPA pamphlet) was provided to occupants/buyers *
- Summary of hazards and recommended controls
