- Sprinkler system type *
- Date of last full inspection and test
- Monitoring/central station is confirmed active *
Sprinkler Control Valves and Piping
- All control valves are open, sealed/supervised, and accessible *
- System and supply gauges read within normal pressure range *
- No corrosion, leaks, mechanical damage, or external loading on piping *
- Sprinkler heads are unobstructed with 18 inches of clearance below and no paint or corrosion *
- Spare sprinkler head cabinet and wrench are present *
- Photo of the riser and gauges
Water Supply and Flow Test
- Main drain test performed and static/residual pressures recorded *
- Inspector's test valve activates the waterflow alarm within 90 seconds *
- Fire department connection (FDC) caps, threads, and check valve are intact *
- Fire pump (if present) started and ran without issue
Fire Alarm Control and Devices
- Fire alarm control panel shows normal status with no trouble or supervisory signals *
- Secondary/battery power is within date and tested under load *
- Manual pull stations and initiating devices are accessible and functional *
- Smoke and heat detectors are within their service and sensitivity interval *
Notification and Egress
- Audible and visible (strobe) notification appliances activate throughout the coverage area *
- Alarm signal transmits to the monitoring station and, where required, the fire department *
- Exit signs and emergency lighting operate on the test
Deficiencies and Sign-Off
- Deficiencies, impairments, or code non-conformities noted
- Overall system condition *
- Inspector name and license/certification *
- Inspector signature *
