General & System Data

  • Levee system/segment identifier (National Levee Database ID) *
  • Is the levee enrolled and inspected under the required routine interval? *
  • Was the operation and maintenance manual reviewed prior to inspection?
  • Date of inspection *

Embankment & Vegetation

  • Is the levee crown/embankment free of settlement, depressions, or loss of freeboard? *
  • Are slopes free of slides, sloughing, erosion, or animal burrows? *
  • Is the vegetation-free zone maintained (no woody vegetation within limits)? *
  • Is there evidence of seepage, sand boils, or wet areas at the landside toe? *
  • Are encroachments (structures, excavations) present within the levee prism?

Floodwalls & Closures

  • Are concrete/steel floodwalls free of cracking, spalling, tilting, or displacement? *
  • Are monolith joints and waterstops intact?
  • Are closure structures (gates, stoplogs) complete, stored, and operable? *
  • Are penetrations through the levee sealed and in good condition?

Drainage & Interior Systems

  • Are drainage ditches, culverts, and gravity outlets clear and functional? *
  • Are flap gates and check valves seating and operable? *
  • Are relief wells, if present, functioning and free of clogging?
  • Is the interior drainage pump station operational and maintained?

Rating & Findings

  • Overall segment rating *
  • Are any unacceptable items requiring corrective action documented? *
  • Photograph representative conditions and deficiencies
  • Inspector signature *
