Dive Setup & Qualifications

  • Is the underwater inspection within the assigned interval (max 60 months or risk-based)? *
  • Is the dive team qualified for the assigned bridge inspection dive level (I/II/III)? *
  • Was a dive safety plan and surface communication in place before entry? *
  • Date of inspection *
  • Water depth and estimated visibility at time of dive

Channel & Streambed

  • Channel and channel protection condition rating (0-9) *
  • Is the streambed profile consistent with prior soundings without new scour holes? *
  • Is channel protection (riprap, articulated block, gabions) in place and stable?
  • Is debris accumulation or channel migration threatening the structure?

Submerged Substructure

  • Are submerged pier and abutment surfaces free of undermining or exposed footings? *
  • Is concrete free of significant deterioration, spalling, or exposed reinforcement below the waterline? *
  • Are timber/steel piles free of marine borer damage, corrosion, or section loss at the mudline?
  • Measured scour depth at deepest point relative to footing (feet)

Scour Evaluation & Plan of Action

  • Scour critical bridges rating (Item 113) confirmed consistent with field findings? *
  • Is a Plan of Action (POA) on file and current for scour-critical structures? *
  • Do observed conditions warrant a POA update or interim countermeasure?

Findings & Documentation

  • Are any critical findings requiring immediate notification present? *
  • Underwater photograph/video of representative conditions captured?
  • Dive team leader signature *
