BMP Identification
- BMP ID / location *
- Date of inspection *
- BMP type (bioretention, permeable pavement, bioswale, green roof, tree box)
Inflow & Pretreatment
- Are inlets, forebays, and pretreatment devices free of clogging or sediment overload? *
- Is flow entering the BMP as designed (no bypass or erosion at inflow points)? *
- Estimated sediment accumulation depth (inches)
Infiltration & Media
- Does the BMP drain down within the design time (no prolonged standing water)? *
- Is permeable pavement surface free of clogging (infiltration confirmed)?
- Is the engineered media/mulch layer intact and at proper depth?
- Are underdrains and cleanouts functioning?
Vegetation & Erosion
- Is vegetation healthy with target coverage and free of invasive species? *
- Is the BMP free of erosion, scour, or channelization? *
- Is trash and debris removed from the surface?
Outflow & Compliance
- Are overflow/outlet structures intact and unobstructed? *
- Is the BMP performing its water-quality/quantity function per the SWMP? *
- Maintenance actions performed / recommended
