- Sign structure type *
- Structure identifier and roadway location *
- Was the previous inspection report and any known fatigue history reviewed?
- Date of inspection *
Truss & Support Members
- Are chord and web members free of cracking, section loss, or impact damage? *
- Are uprights/columns free of corrosion, denting, or distortion? *
- Is the structure free of excessive vibration or sag under wind/traffic?
- Are U-bolts, clamps, and member connections tight?
Connections & Fatigue Details
- Are welded connections (especially tube-to-transverse plate) free of fatigue cracking? *
- Are bolted flange/splice connections complete and tight? *
- Were fatigue-sensitive welds examined closely (or NDT'd) where cracking is suspected?
Base, Anchorage & Foundation
- Are anchor bolts present, tight, and free of corrosion or cracking? *
- Is the base plate free of cracking and fully bearing? *
- Is the foundation free of cracking, spalling, tilting, or undermining? *
Sign Panels & Findings
- Are sign panels, walkways, and attachment hardware secure? *
- Is the maintenance walkway/handrail, if present, safe?
- Are any critical structural findings requiring immediate action present? *
