Surface Condition & Trip Hazards
- Is the walking surface free of vertical displacements exceeding 1/4 inch (or beveled if 1/4-1/2 inch)? *
- Is the surface free of open joints, gaps, or grate openings greater than 1/2 inch? *
- Is the sidewalk free of severe cracking, spalling, settlement, or heaving? *
- Is the surface free of ponding or drainage problems?
- Estimated maximum vertical displacement (inches)
Accessible Route & Width
- Is a continuous clear width of at least 4 feet (48 inches) maintained? *
- Is the running slope within acceptable limits for the pedestrian access route? *
- Is the cross slope 2 percent or less? *
- Is the route free of protruding objects and vertical obstructions?
Curb Ramps
- Is a curb ramp present at the crossing where required? *
- Is the ramp running slope 8.3 percent (1:12) or less? *
- Are detectable warning surfaces (truncated domes) present, contrasting, and correctly placed? *
- Is a level landing (max 2% in all directions) provided at the top? *
- Are flares/side slopes within limits where a pedestrian may walk across them?
Rating & Findings
- Assign a condition/priority rating (1-5)
- Is the deficiency a safety hazard requiring near-term repair? *
- Photograph the defect with a scale reference
- Inspector signature *
