Setup & Identification

- Was confined-space entry procedure followed for interior (Level 2) inspection?

Frame, Cover & Chimney
- Is the cover/lid secure, seated, and free of missing pieces or unsafe defects? *
- Is the frame free of offset, cracking, or loss of seal to the structure? *
- Is the chimney/adjustment ring free of cracks, misalignment, or infiltration? *
- Is the cover free of inflow (holes/pick holes/dished) contributing I/I?
Cone, Wall & Joints
- Are cone and wall free of cracks, spalling, or exposed/corroded reinforcement? *
- Is there evidence of active infiltration at joints, lifting holes, or defects? *
- Are steps/ladder rungs, if present, secure and safe?
- Is corrosion (H2S/microbial) attack evident on concrete surfaces?
Bench & Invert
- Is the bench free of deterioration and shaped to drain to the channel? *
- Is the invert/channel smooth, aligned, and free of blockage or deposits? *
- Are all connecting pipes seated and free of protruding taps or displacement?
Grading & Findings
- Overall structural grade (1-5)
- Overall O&M grade (1-5)
- Photograph frame, interior, and invert
- Inspector signature *
