Wall Identification

  • Wall asset ID / location *
  • Wall type (gravity / cantilever / MSE / soil nail / sheet pile / crib) *
  • Approximate maximum exposed height (ft)

Wall Face Condition

  • Is the wall face free of cracking, spalling, or panel damage? *
  • Are MSE facing panels free of differential movement, gaps, or joint separation?
  • Is exposed reinforcement, connection hardware, or coping corroded/damaged?

Movement & Stability

  • Is the wall free of bulging, leaning, or out-of-plumb rotation? *
  • Is there evidence of settlement, heave, or foundation movement? *
  • Is there evidence of global instability (slope movement, tension cracks behind the wall)? *
  • Measured wall lean / offset from vertical, if surveyed (in)

Drainage

  • Are weep holes / drains open and flowing (not clogged)? *
  • Is there evidence of water staining, efflorescence, or soil piping through the face?
  • Is surface drainage above the wall directed away from the backfill?

Findings

  • Overall wall condition rating (0-9)
  • Is monitoring, repair, or engineering evaluation recommended? *
  • Inspector signature *
