Crossing Identification
- USDOT crossing inventory number *
- Date of inspection *
- Roadway name and railroad/owner
Active Warning Devices
- Do flashing-light signals, bells, and gates activate and operate correctly on train approach? *
- Are warning times consistent with requirements (adequate advance activation)? *
- Are gate arms, lights, and reflectorization undamaged and visible? *
- Is standby power/battery backup functional?
Passive Signs & Markings
- Are crossbucks, advance warning signs, and STOP/YIELD signs present and legible? *
- Are pavement markings (RxR symbol, stop line, no-passing) present and visible?
- Are dynamic envelope / do-not-stop-on-tracks markings provided where warranted?
Surface & Geometry
- Is the crossing surface smooth, intact, and flush with the rails? *
- Is the roadway approach grade/hump acceptable for low-clearance vehicles?
- Is drainage adequate with no ponding or flangeway obstruction?
Sight Distance & Findings
- Is corner/approach sight distance to approaching trains adequate and free of obstructions? *
- Were any safety deficiencies identified? *
