Element Identification

  • NBI structure number *
  • Date of inspection *
  • Prestressing type (pretensioned girders, internal PT, external PT, segmental)
  • Are as-built tendon layout/duct drawings available for reference?

Concrete & Cracking

  • Are prestressed girders/beams free of longitudinal cracking along tendon paths? *
  • Are anchorage zones and end blocks free of cracking or spalling? *
  • Are precast girders free of transverse cracks indicating prestress loss? *
  • Is there rust staining along the soffit or web indicating strand corrosion?

Ducts, Grouting & Voids

  • Are external tendon ducts (HDPE/steel) free of cracking, UV degradation, or splits? *
  • Were grout voids or soft grout investigated at high points and anchorages where suspected?
  • Was NDT (impact-echo, GPR, borescope) used to assess duct filling where warranted?
  • Is there evidence of water intrusion into ducts at low points?

Anchorages & Deviators

  • Are anchorage caps/pour-backs intact and sealed against moisture? *
  • Are deviator blocks and saddles free of cracking or distress?
  • Are exposed anchor heads, wedges, and strand tails free of corrosion? *

Assessment

  • Element condition state (1-4)
  • Is specialized NDT or a detailed tendon investigation recommended? *
  • Notes and recommended follow-up
