Deck & Slab Condition
- Are floor slabs free of significant cracking, delamination, or spalling? *
- Was delamination survey (chain drag/sounding) performed on representative bays?
- Is there evidence of chloride-induced reinforcement corrosion (rust staining, section loss)? *
- Estimated percent of deck area delaminated/deteriorated
Structural Members & Connections
- Are beams, girders, and columns free of structural cracking or distress? *
- Are precast connections, bearing pads, and welded plates free of corrosion or displacement? *
- Are expansion joints functioning and watertight? *
- Are post-tensioning anchorages, if present, free of corrosion or distress?
Durability Protection & Drainage
- Is the traffic-bearing/waterproofing membrane intact where present?
- Are sealant joints and crack sealants in serviceable condition?
- Do floor drains and slopes carry water off without ponding? *
- Is efflorescence/leakage evident at slab underside or construction joints?
Life Safety & Findings
- Are vehicle barriers, guards, and wheel stops secure and adequate? *
- Are stairs, lighting, and pedestrian paths in safe condition?
- Assign an overall condition rating (1-5)
