Structure Identification

  • Sign structure inventory ID *
  • Date of inspection *
  • Structure type (cantilever, span, bridge-mounted, monotube) *
  • Is the original design/shop drawing available for connection detail reference?

Anchor Bolts & Base Connection

  • Are anchor bolts free of visible cracking, section loss, or fracture? *
  • Are all leveling and top nuts present, snug, and properly torqued with no gap under the base plate that should be grouted? *
  • Is the base plate free of cracking at the column-to-base-plate weld toe? *
  • Is the grout pad and foundation free of spalling, cracking, or settlement?

Connection Welds & Fatigue Details

  • Are mast-arm-to-column and truss chord connection welds free of cracking? *
  • Are handhole and access-door reinforcement welds crack-free? *
  • Are U-bolt, clamp, and gusset connections tight and free of cracking or elongated holes?
  • Was NDT (dye penetrant / ultrasonic) performed on suspect fatigue welds?

Members, Sign Panels & Vibration

  • Are truss/chord/post members free of buckling, impact damage, or corrosion section loss? *
  • Are sign panels, walkways, and attachments securely fastened with no loose hardware? *
  • Is the structure free of excessive galloping/vortex-induced vibration signs (fretting, polished contact areas)?
  • Are vibration mitigation devices (dampers) present and functional where required?

Rating & Actions

  • Overall condition rating (0-9 NBI scale)
  • Are urgent (critical fatigue) findings requiring immediate action present? *
  • Recommended inspection interval (months)
  • Findings and recommended actions
