Inspection Setup
- Is the routine tunnel inspection within the required interval (max 24 months)? *
- Is the Team Leader qualified per NTIS personnel requirements? *
- National Tunnel Inventory (NTI) tunnel number *
Structural / Liner
- Is the tunnel liner free of cracking, spalling, or water infiltration beyond documented limits? *
- Interior tunnel liner condition rating (0-9 or element condition state)
- Are portals and roof/ceiling systems structurally sound? *
- Is groundwater control / drainage functioning without ponding on the roadway? *
Ventilation System
- Are supply and exhaust fans operational and free of excessive vibration or corrosion? *
- Are dampers, jet fans, and ductwork intact and securely anchored?
- Are CO / air-quality sensors calibrated and functional?
Fire & Life Safety
- Are fire detection and suppression / standpipe systems present and functional? *
- Are emergency egress / cross-passages unobstructed and signed? *
- Are emergency communication and public-address systems operational?
- Are emergency generators / backup power tested and operational? *
Lighting & Electrical
- Are normal and emergency lighting levels adequate through the tunnel? *
- Are electrical rooms, conduits, and cable trays free of water intrusion and damage?
- Are signs, lane control signals, and variable message signs operational and legible?
Findings & Reporting
- Are any critical findings present requiring immediate notification? *
- Summary of deficiencies and recommended actions
- Team Leader signature *
