Wall Identification

  • Wall ID / location *
  • Date of inspection *
  • Wall type (precast panel MSE, MBW block, RSS, soil-nail shotcrete, etc.)
  • Approximate wall height (ft)

Facing Condition

  • Are facing panels/blocks free of cracking, spalling, or breakage? *
  • Are panel joints free of excessive opening, offset, or spalling? *
  • Is joint filter fabric intact with no soil piping through joints? *
  • Is shotcrete facing (soil-nail walls) free of cracking, delamination, or rust staining at nail heads?

Alignment & Movement

  • Is the wall free of bulging, leaning, or out-of-plumb movement beyond tolerance? *
  • Is the wall alignment free of differential settlement or vertical offsets? *
  • Are there signs of global instability (tension cracks, bulging at toe, heave)? *
  • Maximum measured bulge or offset (inches)

Drainage & Coping

  • Are wall drainage outlets and weep holes flowing and unclogged? *
  • Is the coping/parapet and top-of-wall barrier intact?
  • Is surface drainage directed away from the reinforced zone?
  • Is there evidence of erosion or scour at the wall toe?

Assessment

  • Overall wall condition rating
  • Is instrumentation monitoring or geotechnical evaluation recommended? *
  • Notes and recommendations
