General & Operation

  • Is the inspection within the assigned NBIS interval? *
  • Was a full test opening/closing observed during the inspection? *
  • Movable bridge type *
  • Does the span open and seat smoothly without binding, jerking, or abnormal noise? *
  • Date of inspection *

Structural & Balance

  • Are counterweights, trunnions, and support structure free of cracking or section loss? *
  • Is the span reasonably balanced without excessive motor current or drift? *
  • Are live-load shoes, locks, and centering devices seating correctly? *
  • Are span guides and end lifts adjusted and functioning?

Mechanical Systems

  • Are open gears, pinions, and racks free of excessive wear, pitting, or misalignment? *
  • Are shafts, bearings, and couplings secure and adequately lubricated? *
  • Are span locks and tail locks engaging and disengaging fully? *
  • Are brakes (motor and machinery) adjusted and holding? *
  • Note any abnormal wear, backlash, or lubrication deficiencies

Electrical & Controls

  • Are motors, drives, and control panels free of overheating, corrosion, or loose connections? *
  • Are limit switches and position indicators calibrated and functioning? *
  • Do interlocks prevent operation until traffic gates and barriers are set? *
  • Is backup/emergency power available and tested?

Traffic Control & Findings

  • Do warning gates, barrier gates, signals, and bells operate in the correct sequence? *
  • Are navigation lights and marine signals functioning?
  • Are any critical findings present requiring immediate notification? *
  • Inspector signature *
