General & Identification
- Pole/tower type *
- Structure identifier and location *
- Was the previous inspection report reviewed?
- Date of inspection *
Pole & Structure
- Is the pole/shaft free of cracking, denting, corrosion, or section loss? *
- Are welds at the base plate and shaft splices free of cracking? *
- Is the pole plumb and free of excessive lean?
- Are handhole reinforcements and covers intact?
Base, Anchorage & Foundation
- Are anchor bolts, nuts, and leveling nuts present, tight, and free of corrosion? *
- Is the base plate free of cracking and fully bearing? *
- Is the concrete foundation free of cracking, spalling, or undermining? *
- Is the grout/leveling arrangement per detail (not fully grouted where a gap is required)?
Luminaire & Lowering System
- For high-mast towers, does the lowering (ring/winch) system operate correctly?
- Are luminaires, brackets, and arms secure and undamaged? *
- Are wire ropes, connectors, and hoist components free of wear or corrosion?
- Are luminaires functioning (illuminated at night test or lamp check)?
Electrical & Findings
- Is wiring, grounding, and the pole hand-hole splice compartment safe and weather-tight? *
- Are breakers/fuses and the service point in serviceable condition?
- Photograph base, connections, and any defects
- Inspector signature *
