Run Identification
- Barrier run ID / location (route & station) *
- Barrier type (W-beam / thrie-beam / cable / concrete / bridge rail) *
- Inspection date *
Rail Element Condition
- Is the rail free of significant crushing, tearing, flattening, or corrosion? *
- Are splice bolts and post bolts present and tight? *
- Is the rail free of missing sections or unrepaired prior impact damage? *
- Measured rail top height above pavement/ground (in)
Posts & Blockouts
- Are posts vertical, undamaged, and adequately embedded (not heaved or leaning)? *
- Are blockouts/offset blocks in place and undamaged?
- Is soil at post lines free of erosion, washout, or lack of support?
End Terminals & Transitions
- Is the end terminal a crashworthy, currently accepted (MASH/NCHRP 350) design and undamaged? *
- Is the terminal properly anchored with correct offset and grading? *
- Are transitions to rigid barriers/bridge rail complete and undamaged (no gaps for snagging)? *
Findings
- Does damage warrant repair or replacement? *
- Repair priority (routine / high / emergency for failed end terminal or gap)
- Photograph any impact damage
