Monitoring Setup
- Project / site name *
- Date of reading *
- Instrument type(s) read this visit
- Is the instrumentation and monitoring plan (thresholds, frequency) referenced? *
Instrument Condition
- Are instrument enclosures, caps, and reference points intact and undisturbed? *
- Are readout units/dataloggers calibrated and functioning? *
- Are survey control/benchmarks stable and verified?
- Is automated data acquisition (if used) transmitting valid data?
Data Collection
- Were inclinometer profiles read in both A and B axes with checksums verified?
- Were piezometer water levels/pressures recorded?
- Were settlement/heave and crack-gauge measurements recorded?
- Latest reading value (units per instrument)
Data Review & Thresholds
- Are current readings within established alert/action thresholds? *
- Do trends show accelerating movement or pore-pressure rise? *
- Was any threshold exceedance reported per the notification protocol?
Findings
- Is continued/increased monitoring or engineering review recommended? *
- Notes and observations
