Setup & Qualifications
- Is the inspection led by a Qualified Exterior Wall Inspector (QEWI)? *
- Does the scope include at least one physical (hands-on) examination location per elevation as required? *
- Building identification and cycle number *
- Date of inspection *
Masonry & Wall Surfaces
- Is the masonry/cladding free of cracking, bulging, or displacement? *
- Are mortar joints, sealants, and expansion joints sound? *
- Is spalling or delamination of stone/concrete/terra cotta present? *
- Is there evidence of water infiltration, efflorescence, or freeze-thaw damage?
Anchorage & Attachments
- Are cladding anchors, relieving angles, and shelf angles free of corrosion or failure? *
- Are lintels and window/door heads free of deterioration?
- Are parapets, copings, and cornices stable and adequately anchored? *
- Are balcony/railing attachments secure?
Appurtenances
- Are signs, lighting, and building-mounted equipment securely attached? *
- Are fire escapes, if present, free of severe corrosion and structurally sound?
- Are window guards, A/C units, and flower boxes secured against falling?
Classification & Reporting
- Overall facade condition classification *
- Have any Unsafe conditions been protected (sidewalk shed/netting) and reported to DOB? *
- Photograph representative and defective conditions
- QEWI signature/seal *
