Embankment & Side Slopes
- Is the embankment free of erosion, rills, animal burrows, or seepage? *
- Are side slopes stable and vegetated with no slumping or bare areas? *
- Is the embankment crest free of settlement or cracking?
- Is woody vegetation absent from the embankment (per dam-safety practice)?
Outlet & Spillway
- Is the principal outlet/riser structure free of blockage, corrosion, or damage? *
- Is the trash rack/anti-vortex device intact and clear? *
- Is the emergency spillway free of erosion and obstruction? *
- Is the outfall/energy dissipator free of scour or undermining?
Sediment & Water Quality
- Is sediment accumulation below the cleanout threshold in the forebay and pond? *
- Measured sediment depth in forebay (inches)
- For wet ponds, is the permanent pool at design elevation (no drawdown/clogging)?
- Is the pond free of nuisance conditions (algae, odor, debris)?
Findings
- Were any maintenance deficiencies identified? *
- Maintenance actions recommended
