Structure Identification

  • Culvert asset ID / location *
  • Culvert material (RCP / CMP / concrete box / plastic / masonry) *
  • Span / diameter (ft)
  • Does the culvert meet the 20 ft span threshold requiring NBIS bridge-level inspection?

Barrel Condition

  • Is the barrel free of significant cracking, spalling, or exposed reinforcement (concrete)?
  • Is the invert free of corrosion perforation or section loss (metal culverts)?
  • Is the barrel free of deflection, shape distortion, or partial collapse? *
  • Are joints free of separation, misalignment, or infiltration of backfill soil? *
  • Overall culvert condition rating (0-9)

End Treatments

  • Are headwalls, wingwalls, and aprons intact and stable? *
  • Is there scour or undermining at the inlet or outlet? *
  • Are end sections free of settlement or separation from the barrel?

Hydraulic Performance

  • Is the barrel free of debris, sediment, or blockage restricting flow? *
  • Percent of barrel cross-section obstructed by sediment/debris (%)
  • Is there evidence of overtopping or roadway embankment erosion?
  • Is the channel aligned and stable upstream and downstream?

Findings

  • Is repair, lining, or replacement recommended? *
  • Photograph inlet, outlet, and worst barrel defect
  • Inspector signature *
