Device Identification

  • Attenuator location / asset ID *
  • Date of inspection *
  • Device type (sand barrel array, redirective, non-redirective, gating) *
  • Is the device a MASH-accepted system with a valid FHWA eligibility letter? *

Impact History & Condition

  • Is the device free of recent impact damage requiring reset or replacement? *
  • Are all energy-absorbing cartridges/cells/sand modules present and intact? *
  • Are fender panels, diaphragms, and side panels undamaged and aligned?
  • Number of modules/bays damaged or missing

Anchorage & Backup

  • Is the concrete pad/foundation free of cracking or spalling? *
  • Are all anchor bolts present, tight, and free of corrosion? *
  • Is the backup structure and transition to the shielded hazard properly connected? *
  • Is the monorail/guidance track (if present) straight and unobstructed?

Delineation & Actions

  • Are reflective markers/nose delineation present and visible at night?
  • Is the device free of debris, vegetation, or drainage ponding at its base?
  • Is repair, reset, or full replacement required? *
  • Findings and recommended actions
