Impact History & Condition
- Is the device free of recent impact damage requiring reset or replacement? *
- Are all energy-absorbing cartridges/cells/sand modules present and intact? *
- Are fender panels, diaphragms, and side panels undamaged and aligned?
- Number of modules/bays damaged or missing
Anchorage & Backup
- Is the concrete pad/foundation free of cracking or spalling? *
- Are all anchor bolts present, tight, and free of corrosion? *
- Is the backup structure and transition to the shielded hazard properly connected? *
- Is the monorail/guidance track (if present) straight and unobstructed?
Delineation & Actions
- Are reflective markers/nose delineation present and visible at night?
- Is the device free of debris, vegetation, or drainage ponding at its base?
- Is repair, reset, or full replacement required? *
