Survey Setup

  • Structure / element ID *
  • Date of survey *
  • Element type (deck, girder, column, wall, foundation) *
  • Is a crack map / grid reference established for documentation? *

Crack Characterization

  • Are crack widths measured with a crack comparator or microscope? *
  • Maximum measured crack width (inches)
  • Are cracks classified by orientation (transverse, longitudinal, diagonal, map/pattern)? *
  • Are cracks assessed as active (moving) vs. dormant?

Cause Assessment

  • Is the likely cause identified (structural load, shrinkage, thermal, ASR, corrosion, settlement)? *
  • Is there evidence of reinforcement corrosion (rust staining, delamination)? *
  • Is efflorescence, leaching, or moisture migration present along cracks?
  • Was petrographic sampling (ASTM C856) recommended for suspected ASR/DEF?

Severity & Actions

  • Do any cracks exceed the tolerable width for the exposure condition? *
  • Is the crack pattern indicative of a structural (versus durability) concern? *
  • Is repair (epoxy injection, routing/sealing, overlay) recommended?
  • Findings and recommended actions
