Structure & Retrofit Scope

  • NBI structure number *
  • Date of inspection *
  • Retrofit measures present (restrainers, jackets, isolation bearings, shear keys, catchers) *
  • Are as-built retrofit drawings available for reference?

Restrainers & Connections

  • Are cable/rod restrainers intact, properly tensioned, and free of corrosion? *
  • Are restrainer anchorages, brackets, and bolts sound with no cracking? *
  • Are seat extenders / catcher blocks in place and undamaged?
  • Is the available support/seat length free of degradation?

Bearings & Isolation

  • Are isolation/elastomeric bearings free of excessive deformation, bulging, or bond failure? *
  • Are shear keys/lugs intact with no cracking or spalling of surrounding concrete? *
  • Are bearing anchor bolts and keeper assemblies intact and tight?

Columns & Substructure Retrofit

  • Are steel/composite column jackets free of buckling, corrosion, or debonding? *
  • Is the annular grout between jacket and column intact?
  • Are footing/cap-beam retrofits (overlays, prestressing) free of distress?

Rating & Actions

  • Are retrofit components functioning as intended with no critical deficiencies? *
  • Retrofit condition rating (0-9)
  • Findings and recommended actions
