- Date of inspection *
- Is the bridge on the Scour Plan of Action (POA) / scour-critical list? *
- Approximate flow condition at inspection (dry, low, moderate, high)
Channel & Streambed
- Is the channel free of significant aggradation, degradation, or lateral migration? *
- Is the streambed at piers and abutments free of new scour holes? *
- Measured/probed scour depth at critical pier (feet below reference)
- Is debris accumulation at piers minimal (not constricting flow)?
Foundation Exposure
- Are pier and abutment footings/piles free of undermining or exposure? *
- Are spread footings still fully embedded to design elevation?
- Is there any evidence of substructure settlement, tilting, or rotation? *
Countermeasures
- Is placed riprap adequately sized, graded, and in position (not displaced or launched away)? *
- Are articulated concrete blocks/mats, gabions, or grout bags intact?
- Are guide banks, spur dikes, or bank revetments stable and functional?
- Is the filter/geotextile layer intact where observable?
Rating & Actions
- Channel/scour condition rating (0-9, NBI Item 61/113 basis)
- Is a POA action (dive inspection, monitoring, closure) triggered? *
- Findings and recommended actions
