Site & Waterway

  • NBI structure number *

  • Approximate flow condition at inspection (dry, low, moderate, high)

Channel & Streambed

  • Is the channel free of significant aggradation, degradation, or lateral migration? *
  • Is the streambed at piers and abutments free of new scour holes? *
  • Measured/probed scour depth at critical pier (feet below reference)
  • Is debris accumulation at piers minimal (not constricting flow)?

Foundation Exposure

  • Are pier and abutment footings/piles free of undermining or exposure? *
  • Are spread footings still fully embedded to design elevation?
  • Is there any evidence of substructure settlement, tilting, or rotation? *

Countermeasures
