Rating Currency
- Structure number *
- Does the bridge have a current load rating for the required legal and design loads? *
- Was the load rating re-evaluated after the most recent inspection findings or condition change? *
- Date of most recent load rating
Rating Inputs & Method
- Is the rating method (LRFR / LFR / ASR) documented and appropriate for the structure? *
- Do span geometry, member sizes, and material properties in the rating match as-built/plan data? *
- Are dead load and wearing surface assumptions consistent with current field conditions?
Condition-Based Adjustments
- Are section loss, deterioration, and defects from the latest inspection reflected in member capacity? *
- Are condition/system factors applied consistent with observed member condition states?
- Governing rating factor (RF) for the controlling member/load
Posting & Restrictions
- If the rating requires it, is the bridge posted for load limits and are signs installed? *
- Are posted load limits consistent between the rating, NBI records, and field signage? *
- Is a permit-vehicle review process in place for loads exceeding legal limits?
Documentation
- Are load rating calculations sealed by a licensed professional engineer and archived? *
- Are rating results, dates, and assumptions recorded in the bridge management/NBI system? *
