Deck Identification

  • NBI structure number *
  • Date of inspection *
  • Overlay/wearing surface type (asphalt, latex-modified concrete, polymer, etc.)
  • Is the membrane/waterproofing system type documented?

Overlay Surface Condition

  • Is the overlay free of rutting, shoving, raveling, or potholing? *
  • Is the overlay free of transverse/longitudinal reflective cracking? *
  • Estimated percent of deck area with distress
  • Is the riding surface free of ponding or improper cross-slope drainage?

Delamination & Bonding

  • Was chain drag or hammer sounding performed to detect delamination? *
  • Is the overlay well bonded and free of debonding/delamination between overlay and deck? *
  • Estimated delaminated area (sq ft)
  • Are core samples or GPR recommended to confirm subsurface condition?

Waterproofing & Moisture Intrusion

  • Is the underside of the deck free of efflorescence, leaching, or active leakage? *
  • Are membrane terminations at curbs, joints, and scuppers intact?
  • Is deck reinforcement free of exposure or corrosion staining visible from below? *
  • Is there evidence of chloride penetration or map cracking?

Assessment

  • NBI deck condition rating (0-9)
  • Is overlay replacement or rehabilitation recommended within the cycle? *
  • Notes and recommendations
