Deck Identification
- NBI structure number *
- Date of inspection *
- Overlay/wearing surface type (asphalt, latex-modified concrete, polymer, etc.)
- Is the membrane/waterproofing system type documented?
Overlay Surface Condition
- Is the overlay free of rutting, shoving, raveling, or potholing? *
- Is the overlay free of transverse/longitudinal reflective cracking? *
- Estimated percent of deck area with distress
- Is the riding surface free of ponding or improper cross-slope drainage?
Delamination & Bonding
- Was chain drag or hammer sounding performed to detect delamination? *
- Is the overlay well bonded and free of debonding/delamination between overlay and deck? *
- Estimated delaminated area (sq ft)
- Are core samples or GPR recommended to confirm subsurface condition?
Waterproofing & Moisture Intrusion
- Is the underside of the deck free of efflorescence, leaching, or active leakage? *
- Are membrane terminations at curbs, joints, and scuppers intact?
- Is deck reinforcement free of exposure or corrosion staining visible from below? *
- Is there evidence of chloride penetration or map cracking?
Assessment
- NBI deck condition rating (0-9)
- Is overlay replacement or rehabilitation recommended within the cycle? *
