WHO Five Moments
- Was hand hygiene performed BEFORE touching a patient? *
- Was hand hygiene performed BEFORE a clean/aseptic procedure? *
- Was hand hygiene performed AFTER body fluid exposure risk? *
- Was hand hygiene performed AFTER touching a patient? *
- Was hand hygiene performed AFTER touching patient surroundings? *
Technique & Product
- Was alcohol-based hand rub used for at least the manufacturer-specified contact time and full coverage? *
- Was soap and water used when hands were visibly soiled or after C. difficile/norovirus contact? *
- Were fingernails kept short and artificial nails avoided by staff with direct patient contact? *
Glove Use & Program
- Were gloves removed and hand hygiene performed between patients (gloves not a substitute for hand hygiene)? *
- Are trained observers using a standardized, non-punitive observation tool? *
- Is compliance data aggregated and fed back to units with improvement goals? *
Access & Infrastructure
- Are alcohol-based hand rub dispensers accessible at the point of care and at room entrances? *
- Are dispensers monitored for function and refilled promptly when empty? *
- Is a skin-care program available to reduce dermatitis that discourages hand hygiene?
