- Date of survey *
- Antenna frequency used (MHz)
- Is traffic control in place per the approved MOT plan? *
Calibration & Acquisition
- Is the GPR system calibrated and depth/velocity verified for the deck? *
- Are survey lines/grid spacing consistent with specified coverage? *
- Line/grid spacing used (feet)
- Is positional referencing (DMI/GPS/station marks) recorded?
Data Interpretation
- Are areas of top-mat rebar amplitude attenuation (deterioration) mapped? *
- Estimated percent deck area deteriorated (%)
- Is measured concrete cover over top reinforcement within expected range?
- Are results corroborated with chain-drag / impact-echo where available?
Reporting & Actions
- Is a deterioration contour/quantity map produced for repair planning? *
- Is deck rehabilitation or overlay quantity estimated from the scan?
- Findings and recommended actions
