Program & Policy

  • Is a written safe patient handling and mobility policy establishing a minimal manual lift culture in place? *
  • Is there a SPHM committee or program leader with defined accountability? *
  • Are patient-handling injuries to staff tracked and analyzed for improvement? *

Assessment & Equipment

  • Is each patient's mobility and handling needs assessed and the required equipment level documented? *
  • Are mechanical lifts, ceiling lifts, sit-to-stand devices, or friction-reducing devices available and functional on the unit? *
  • Is lift and transfer equipment inspected and maintained per manufacturer schedules? *

Training & Practice

  • Are staff trained and competency-assessed on safe patient handling equipment and techniques? *
  • Is early and progressive mobility promoted with appropriate assistive equipment?
  • Is manual lifting limited to below recommended weight thresholds using assistive devices? *

Evaluation & Improvement

  • Is the SPHM program evaluated at least annually for effectiveness and staff injury trends? *
  • Are near-miss and injury reports used to drive equipment and process improvements? *
  • Are new construction and renovations designed to accommodate lift equipment?
