Facility Identification

- Are above-water and below-water (dive) scopes both addressed? *
Fender System
- Are fender panels, rubber units, and chains intact and properly aligned? *
- Are timber/plastic fender piles free of breakage, marine borer damage, or excessive splitting? *
- Are fender fasteners, bolts, and weld connections sound?
- Is energy-absorbing capacity visibly compromised (crushed/torn units)?
Piles, Dolphins & Substructure
- Are support and mooring/breasting dolphin piles free of corrosion, cracking, or section loss? *
- Are concrete pile caps and deck framing free of spalling and exposed rebar? *
- Is scour or bedding loss present around pile foundations?
- Estimated max pile section loss in splash zone (%)
Transfer Span & Mechanical
- Is the vehicle/passenger transfer span (apron) structurally sound and properly seated? *
- Are lift mechanisms, hydraulics, counterweights, or towers functional and lubricated?
- Are bull rails, cleats, bollards, and mooring hardware secure?
Rating & Actions
- Overall facility condition rating (0-9)
- Are operationally critical deficiencies present? *
