Inspection Setup

  • Is the routine tunnel inspection within the required interval (max 24 months)? *
  • Is the Team Leader qualified per NTIS personnel requirements? *
  • National Tunnel Inventory (NTI) tunnel number *

Structural / Liner

  • Is the tunnel liner free of cracking, spalling, or water infiltration beyond documented limits? *
  • Interior tunnel liner condition rating (0-9 or element condition state)
  • Are portals and roof/ceiling systems structurally sound? *
  • Is groundwater control / drainage functioning without ponding on the roadway? *

Ventilation System

  • Are supply and exhaust fans operational and free of excessive vibration or corrosion? *
  • Are dampers, jet fans, and ductwork intact and securely anchored?
  • Are CO / air-quality sensors calibrated and functional?

Fire & Life Safety

  • Are fire detection and suppression / standpipe systems present and functional? *
  • Are emergency egress / cross-passages unobstructed and signed? *
  • Are emergency communication and public-address systems operational?
  • Are emergency generators / backup power tested and operational? *

Lighting & Electrical

  • Are normal and emergency lighting levels adequate through the tunnel? *
  • Are electrical rooms, conduits, and cable trays free of water intrusion and damage?
  • Are signs, lane control signals, and variable message signs operational and legible?

Findings & Reporting

  • Are any critical findings present requiring immediate notification? *
  • Summary of deficiencies and recommended actions
  • Team Leader signature *
