Program & Management

  • Is there management commitment and a written ergonomics program? *
  • Is there a process for employees to report MSD symptoms and concerns early? *
  • Are OSHA 300 logs and symptom reports reviewed to identify problem jobs? *

Risk Assessment

  • Are jobs assessed for MSD risk factors (force, repetition, awkward postures, vibration, contact stress)? *
  • Are office workstations evaluated for monitor height, chair, keyboard/mouse, and reach? *
  • Are manual material handling tasks assessed for lifting weight, frequency, and origin/destination heights? *

Controls

  • Are engineering controls (adjustable workstations, lift assists, conveyors) implemented where feasible? *
  • Are administrative controls (job rotation, rest breaks, pacing) used to reduce exposure? *
  • Is control effectiveness verified after implementation? *

Training & Follow-up

  • Are employees trained to recognize MSD signs, symptoms, and reporting procedures? *
  • Are supervisors trained to identify risk factors and respond to reports? *
