Application & Inquiries
- Does the file contain a completed employment application with 3-year employment/residence history? *
- Was an inquiry made to previous DOT-regulated employers for the prior 3 years? *
- Is a safety performance history (drug/alcohol, accidents) investigation documented? *
Driving Record
- Was a motor vehicle record obtained from each state of licensure at hire? *
- Is an annual MVR review documented for each driver? *
- Is a driver's annual list of violations (or equivalent) on file? *
Medical Certification
- Is a valid medical examiner's certificate from the National Registry on file? *
- Is the medical certificate current (not expired) and matched to the CDLIS record where applicable? *
- List drivers with medical certificates expiring within 30 days: *
Road Test & Qualification
- Is a road test certificate or an accepted equivalent (valid CDL) on file? *
- Does the file confirm the driver meets the general qualifications (age, language, license)? *
- Are records retained for the required duration during and after employment? *
