Prequalification
- Are contractors evaluated on safety history (EMR, OSHA rates, citations) before selection? *
- Are certificates of insurance and required licenses verified? *
- Are contractor written safety programs reviewed for adequacy? *
Orientation & Communication
- Do contractors receive a site-specific safety orientation covering facility hazards? *
- Are emergency procedures and reporting requirements communicated to contractors? *
- Is a pre-job planning or JHA meeting held before high-hazard work? *
On-Site Oversight
- Are contractor permits (hot work, confined space, LOTO) coordinated with the host employer? *
- Are periodic audits or observations of contractor work performed? *
- Are contractor injuries, illnesses, and incidents on site reported and investigated? *
Performance Evaluation
- Is contractor safety performance evaluated at project completion? *
- Are results used to update the approved contractor list? *
