  • Is a project charter approved with problem statement, goal, scope, and business case? *
  • Are the customer (VOC) and critical-to-quality (CTQ) requirements defined? *
  • Is a high-level process map (SIPOC) completed? *
  • Project name / ID *

Measure

  • Is the process performance baseline established with valid data? *
  • Has a measurement system analysis (MSA) been performed on key metrics? *
  • Is a data collection plan documented and executed? *

Analyze

  • Have potential root causes been identified and validated with data (not opinion)? *
  • Were appropriate statistical tools (hypothesis tests, regression, etc.) applied? *
  • Are the vital few causes distinguished from the trivial many?

Improve

  • Were improvement solutions generated, selected, and piloted? *
  • Is the improvement demonstrated to address the validated root causes? *
  • Was a risk assessment (e.g., FMEA) performed on the proposed solution?

Control

  • Is a control plan established to sustain the gains? *
  • Are process changes documented in standard work and procedures? *
  • Are financial benefits validated and the project formally closed with sign-off? *
  • Validated annual savings
