Define
- Is a project charter approved with problem statement, goal, scope, and business case? *
- Are the customer (VOC) and critical-to-quality (CTQ) requirements defined? *
- Is a high-level process map (SIPOC) completed? *
- Project name / ID *
Measure
- Is the process performance baseline established with valid data? *
- Has a measurement system analysis (MSA) been performed on key metrics? *
- Is a data collection plan documented and executed? *
Analyze
- Have potential root causes been identified and validated with data (not opinion)? *
- Were appropriate statistical tools (hypothesis tests, regression, etc.) applied? *
- Are the vital few causes distinguished from the trivial many?
Improve
- Were improvement solutions generated, selected, and piloted? *
- Is the improvement demonstrated to address the validated root causes? *
- Was a risk assessment (e.g., FMEA) performed on the proposed solution?
Control
- Is a control plan established to sustain the gains? *
- Are process changes documented in standard work and procedures? *
- Are financial benefits validated and the project formally closed with sign-off? *
- Validated annual savings
