Reporting & Immediate Response
- Was the incident reported and recorded in a timely manner? *
- Were immediate actions taken to control and correct the situation and deal with consequences? *
- Was the scene secured and evidence preserved where required?
- Incident date *
Investigation & Root Cause
- Was the incident investigated in a timely manner to determine root cause? *
- Was a structured root cause method (e.g., 5-Why, fishbone, tree analysis) used? *
- Was the need to eliminate the root cause(s) to prevent recurrence evaluated? *
- Were similar incidents, potential incidents, and nonconformities considered? *
Worker Participation & Communication
- Were workers and their representatives involved in the investigation? *
- Were results of the investigation and any actions communicated to relevant workers? *
- Were legal/regulatory reporting obligations met where applicable?
Corrective Action & Effectiveness
- Were corrective actions determined and implemented per the hierarchy of controls? *
- Were OH&S risks and opportunities reassessed as needed? *
- Was the effectiveness of corrective action reviewed? *
- Was documented information retained as evidence of the incident and actions taken? *
