Step 1-2: Planning & Structure Analysis
- Is the DFMEA scope, boundaries, and cross-functional team defined (planning & preparation)? *
- Is the design decomposed into a structure tree (system, subsystem, component) for structure analysis? *
- Are design lessons learned and a baseline/foundation DFMEA used as inputs?
Step 3: Function Analysis
- Are the functions and requirements of each structure element defined? *
- Are function relationships (parameter diagram / function net) established between elements? *
- Are requirements traceable to customer, regulatory, and internal design inputs? *
Step 4-5: Failure & Risk Analysis
- Are failure effects, failure modes, and failure causes identified and linked as a failure chain? *
- Are Severity, Occurrence, and Detection ratings assigned using the applicable rating tables? *
- Are current prevention and detection controls documented for each failure cause? *
- Is Action Priority (High/Medium/Low) determined instead of relying solely on RPN? *
Step 6-7: Optimization & Documentation
- Are optimization actions defined for high and medium Action Priority items with responsibilities and target dates? *
- Are ratings reassessed after actions are implemented and their effectiveness confirmed? *
- Are the results and residual risk communicated and the DFMEA documented and retained? *
